District Choir Festival dress rehearsal permission slips!!!

Hello All,

I am still missing several permission slips for the District Choir Festival dress rehearsal! It is on Wednesday morning, so I need those tomorrow. The students have been given these since before spring break. Make sure the student name is at the top, and the signature is included at the bottom. Feel free to include an emergency contact number at the bottom of the form as well.
